Scenario

A client is post-op day 2 from colorectal surgery with a closed abdominal incision, temp 99.8°F (37.7°C), and WBC mildly elevated.

Nursing statements

Risk for infection related to surgical wound and altered immune response.
Incision will remain clean, dry, and without purulent drainage during hospitalization.
Perform dressing change with sterile technique and document appearance.
Monitor temperature, WBC, and client report of increased pain at site.
Remove dressing and leave incision open to air permanently.
Inspect incision for erythema, warmth, drainage, and separation.
Compare daily temperature and WBC trend to baseline.
