ADPIE Nursing Process

Stroke with Hemiparesis · L1

How this exercise works

Drag each nursing statement into Assessment, Diagnosis, Planning, Implementation, or Evaluation. Only one phase fits each statement — look for data vs. action cues.

Fundamental concept

How systematic ADPIE thinking supports safe practical nursing care, aligned with HHS quality initiatives.

Scenario

A client admitted 24 hours ago with left-sided weakness needs assistance with transfers and has difficulty swallowing thin liquids on bedside swallow screen.

Nursing statements

Drag each statement into the correct ADPIE phase, or into trash if it does not belong.

Allow thin liquids because the client is thirsty.
Implement fall precautions, bed alarm, and upright position for oral intake.
Client will transfer safely with one-person assist within 3 days.
Evaluate ability to perform ADLs and need for assistive devices.
Document improved transfer ability compared to admission.
Assess muscle strength, balance, swallow, and National Institutes of Health Stroke Scale per protocol.
Impaired physical mobility related to neuromuscular impairment.
