Scenario

A client admitted for pancreatitis has fasting glucose 286 mg/dL, polyuria, polydipsia, and acetone breath odor.

Nursing statements

Risk for unstable blood glucose related to insulin deficiency and illness.
Blood glucose will trend toward 140–180 mg/dL with ordered therapy.
Monitor blood glucose per sliding scale orders and assess for ketosis signs.
Evaluate glucose response 2 hours after insulin adjustment.
Review fluid balance, weight, and neurologic status.
Hold all insulin because the client is NPO without provider direction.
Administer insulin and IV fluids per protocol; maintain NPO if ordered.
