ADPIE Nursing Process

Dehydration in Older Adult · L1

How this exercise works

Drag each nursing statement into Assessment, Diagnosis, Planning, Implementation, or Evaluation. Only one phase fits each statement — look for data vs. action cues.

Fundamental concept

How systematic ADPIE thinking supports safe practical nursing care, aligned with HHS quality initiatives.

Scenario

A 79-year-old is admitted with dizziness, dry mucous membranes, poor skin turgor, urine output 20 mL/hr, and sodium 152 mEq/L.

Nursing statements

Drag each statement into the correct ADPIE phase, or into trash if it does not belong.

Administer IV fluids at prescribed rate with pump and monitor for overload.
Client will achieve balanced intake/output and stable vitals within 24 hours.
Review intake/output and serum sodium, BUN, and creatinine.
Monitor vital signs, weight, mucous membranes, and capillary refill.
Deficient fluid volume related to inadequate oral intake and hypernatremia.
Restrict all fluids to prevent further dehydration.
Reevaluate orthostatic vitals and urine output each shift.
