Braden Scale

Young trauma on traction · L1

How this exercise works

Rate each Braden subscale (sensory, moisture, activity, mobility, nutrition, friction). Pick values that match the patient description to estimate pressure-injury risk.

Fundamental concept

Rating sensory perception, moisture, activity, mobility, nutrition, and friction to guide prevention for at-risk patients.

Scenario

A 22-year-old in skeletal traction for a femur fracture is alert with full sensation. He is bedfast but shifts upper body position independently. Skin is dry; he eats all meals delivered. Staff use a lift sheet for lower-body repositioning.

Score Braden subscales for a bedfast but otherwise healthy trauma patient.
