SOAP Exercise

Syncope at work

How this exercise works

Read each report sentence, then drag it into Subjective (what the patient says), Objective (what you measure or observe), Assessment (your clinical impression), or Plan (treatments and transport). Drop irrelevant noise into the trash. Submit when every sentence is placed.

Fundamental concept

How structured Subjective, Objective, Assessment, and Plan charting supports continuity of care and medicolegal accountability.
